At WholeCare Family and Occupational Health, we provide school, sports, camp, and college physicals for children, adolescents, and young adults ages 5 and older. Whether your child is entering elementary school, participating in athletics, attending summer camp, or preparing for college, we perform comprehensive physical examinations, complete required health forms, and help ensure they are ready for the next stage of their education or activities.

Immunization Record Review & Vaccine Referrals
At WholeCare Family & Occupational Health, we carefully review your child's immunization records to help ensure they meet school and state requirements.
While vaccines are not currently administered onsite, we provide personalized vaccine recommendations and referrals to local pharmacies or community vaccination providers when immunizations are needed.

No. WholeCare Family and Occupational Health does not administer vaccines onsite. We review immunization records, discuss recommended immunizations, and provide referrals to local vaccination providers when needed.

If your school, camp, sports organization, or college has provided a specific physical form, please bring it to your appointment. If you don't have the required forms, don't worry. WholeCare Family and Occupational Health has Connecticut State School Health Assessment forms and general physical examination forms available to complete during your visit.

Yes. We complete the Connecticut State School Health Assessment Record, general physical examination forms, and many school, sports, camp, and college health forms. If your organization requires a specific form, simply bring it with you to your appointment.
